Registration Procedure
* by mail (Registration by post or mail order) All payments are to be made in HK Dollars or RMB
* for registration, please complete the form and return and with a cheque of appropriate fee payable to "Macau Surgical Association" to Dr. Lee Man Sang * remit money to "Bank of Communications, Zhuhai Branch" to "Macau Surgical Association" to Dr. Lee Man Sang (china only) * no VISA / Master Card payment is acceptable. * payment should be sent with the completed registration form to the Congress Secretariat.

Cancellation of Registration
Cancellation Policy All Cancellation must be make in writing to the Conference Secretariat.Refunds will be made after the Conference. 50% refund if written confirmation is received on or before May 13, 2008. No refund will be made thereafter.

Official Receipt
An official receipt will be issued upon payment of the registration fee.

Entitlement
Registered delegates will be entitled to: * participate in all scientific sessions * visit the Exhibition * coffee and tea during the morning and afternoon breaks * receive a set of official publications * attend the Opening Ceremony Registered accompanying persons will be entitled to: * attend the Opening Ceremony

Important Notes
Registration forms received without registration fees will not be processed Attend all conference sessions, trade exhibition, coffee breaks, conference bag, program book and certificate of attendance for sessions scheduled for June 13-16, 2008.
